Does My Child Have ASD and ADHD - or Just ASD?

"Is it possible to have a child diagnosed with ADHD, but later discover that he really has ASD? Our son currently has a diagnosis of ADHD, but the more we research, the more we think he has ASD instead. Also, is it possible to have both disorders at the same time?"

Yes, and yes. Even though doctors have clear diagnostic guidelines for children with ASD (High-Functioning Autism), some children get misdiagnosed with other disorders. ADHD and Oppositional Defiant Disorder (ODD) are two diagnoses that children with autism often get diagnosed with. Both diagnoses have features similar to ASD, particularly ADHD.

ADHD is a genetic disorder as is ASD. Both situations involve children with difficulty learning and difficulty with interacting with other children. There can be hyperactivity with both disorders as well. Because of the similarities of the two disorders, the doctor must look at the checklists for both disorders to see which of the two disorders matches the best.

In looking at the two disorder checklists, it’s easy to see that there are similarities and differences...

ASD Behavioral Checklist:
  • Difficulty mixing with other children
  • No real fear of danger
  • Tantrums, displays extreme distress for no apparent reason
  • Inappropriate giggling or laughing
  • May not want cuddling or act cuddly
  • Noticeable physical over-activity or extreme under-activity
  • Little (or no) eye contact
  • Works impulsively, often makes careless mistakes, work is sloppy 
  • Uneven gross/fine motor skills

ADHD Behavioral Checklist:
  • Can't talk or play quietly, disrupts others with talk or actions
  • Difficult awaiting turn in games or activities 
  • Engages in potentially dangerous activities
  • Plays without normal caution or consideration of consequences 
  • Severe temper tantrums 
  • Interrupts, disrupts, talks and acts inappropriately
  • When younger, difficulty accepting soothing or holding
  • Always on the move, overactive, even during sleep
  • Often does not seem to listen when spoken to directly
  • Often does not give close attention to details or makes careless mistakes in school work or other activities
  • Uneven gross/fine motor skills

Using checklists like these may be the best option in diagnosing a child correctly and not missing the diagnosis of ASD when it exists.

When it comes to anxiety-based/type disorders, it's rare for a child to be diagnosed with only one; more often than not, there will be two, three or even more at work at the same time. My son has Asperger's, ADD and Intermittent Explosive Disorder. There might even be more but those are the diagnoses so far...
•    Anonymous said... Unfortunately that's a "go to" diagnosis in public schools. I've seen so many children misdiagnosed as ADHD when there was something else going on. That was what the teachers were suggesting to me also, with my kiddo. Behaviors that would fall under ADHD can be easily found in other diagnosis also, especially with Aspergers.
•    Anonymous said... This appears to be so common - it angers me. I firmly believe that clinical (NHS) staff are told from'up above' to exhaust everything else before diagnosing Aspergers/HFA. Purely because of the money involved once you have that 'Autism' word on paper. I know my son has Aspergers - not ADHD as they have tried to tell us. Don't give up and you are allowed to disagree with them!

•    Anonymous said... That happened to me. My son was given a LIST of issues (at age 5/6), I mean it took the whole page. It just wasn't accurate. We were given (by a different doctor) an aspergers diagnoses about 6 months later. It fit much better and we were able to help him more appropriately.

One thing I've learned about ALL of this stuff is, the diagnostic "labels" are really more useful for determining your eligibility for various types of funding or which programs you can get them into than anything else. The truth is, there's a lot of overlap of signs/symptoms of ADD, ADHD, Asperger's, or "childhood bi-polar". (In fact, depending on who you ask, "childhood bi-polar" doesn't even really exist!) You'll also see such conditions as "separation anxiety disorder" mentioned in some cases. (EG. There was a divorce when the child was young and he/she wound up not seeing one of the parents anymore.) They might even wind up declaring a kid has "behavior disorder, undefined". Ultimately, you have to figure out as a parent what seems to work for your kid and what doesn't, and be flexible enough to keep changing things up as he/she gets older. Our kid went through a phase where, at least according to the school system, ADHD medications were a big help in allowing her to concentrate and succeed in her school-work. But now, I took her off of those meds because I realized her personality and attitude has changed as she's matured. She did just fine last school year without it. Now, her biggest problems seem to be in other areas than concern about hyperactivity or "lack of ability to concentrate". Now, she just has more issues making and keeping friends, and not seeming to understand what's inappropriate to say in some social situations.

Nothing was working until we were able to get his anxiety and adhd under control with meds. They have improved the quality of his life considerably.

•    Anonymous said... My son was diagnosed with both. I know they say you can't really outgrow a diagnosis but, at the age of 13 now, I don't believe the ADHD dx fits him anymore. But yes, it is possible to have both and to get a wrong diagnosis. The Connor chart gave my son a diagnosis of ODD (Oppositional Defiance Disorder) and it was wrong. The test was given right after my ex husband and I got a divorce and my son was lashing out and angry. Shortly after he saw a clinical psychologist who agreed with me that the dx wasn't accurate/fitting for my son.

By the age of 12 after other series of problems with behavior in school and seeing a psychologist after being expelled from school for a month because they refused to accepting him back without knowing what else was wrong with him the psychologist came back not only with the ADHD which fell as the second axis in the diagnosed and aspergers in the first. He also had depression. Short is you can have more than one diagnosis. Now I am proud to say that with therapy and lots of prayer my son is doing much, much better. He rarely exhibits symptoms of depression and he is no longer hyper but does need help with focusing and memory. Extremely forgetful and organized. But if we give them the tools they need they are successful in functioning at school and making some friends. My son is a blessing to my life and I know he was given to me for a special reason. Don't loose hope parents!! Merely watch all the behaviors as they change with age and hormonal changes.
•    Anonymous said... My son has rigidity of thought and struggles with impulse control, both Asperger and ADHD, resulting in no friends and lots of behavior problems. The doctors prescribed 2 medicines for symptoms and the results have been amazing: friends, no negatives marks for first time, A's, able to play with siblings. Even off the medicine, he has ingrained correct behavior and is easier.

As his individual issues were addressed with therapies, his Asperger's symptoms made themselves more evident. It wasnt until he was about 11 that we had him tested for aspergers again with a very strong positive result. I know he still has ADHD and still needs his medication for that. •    Anonymous said... My child has ADHD, ODD and Autism (and several others). We got the ODD and ADHD diagnosis first. Unfortunately a lot of the time you will get one diagnosis at a time. It is actually VERY common for a child who has Autism to also have co-concurring diagnosis... so I wouldn't be surprised if they tell you later on that your child does have another diagnosis on top of the ADHD. We have about 7 diagnosis. The thing is, all they do is build a profile on my daughter and help us treat her better. As overwhelming as that sounds on the surface, it has helped us find better ways to help her and treat her.

I have spoken to the school she is attending and they have said they will be doing an intervention for 6 weeks and see how she is during that timeframe. Has anyone dealt with the school side of dealing with Aspergers and ADHD. She is not on medicine for ADHD, and we are still setting up appointments for her having the possibility of CAPD (Central Auditory Processing Disorder).

Labels are great for school services like social classes, behavior chart, recess monitor, etc., but I am finding a good diagnosis of symptoms and treating the specific symptom through medicine and therapy is really helpful. At first reading everything on the labels seemed overwhelming. Once specialists helped me and school on what was hindering him, that is when progress was made for him.

Always keep in the back of your mind that you are his Mother, have faith in your own instincts and read as much as you can on both subjects. Self information is priceless. No one, not a teacher, not a principal, not a doctor is more of an expert on your child as you are.
•    Anonymous said... I was positive my son was just ADHD.. Because he could communicate and talk so well with us... I didn't realize that in public, at school, etched it was a very different story and that he grew increasingly quiet, and shy. Be ause school kept telling me he was autistic I started spying on him at recess and saw my son sitting on playground not talking to anyone. He was overloaded and didn't know what to do. We started him on counseling after that. He is still shy and inappropriate at times but we are working on it.

•    Anonymous said... As a special ed teacher, for many years I knew my, now 16 yr old daughter had ADD, however for years I also suspected she has Aspergers as well. The older she got the surer I was; many of the Aspie behaviors have become more clearly defined. Finally, about 3 years ago a new therapist my daughter began seeing agreed with me. While it doesn't really change anything it was validation. My husband is still skeptical, but doesn't have the experience I have.

•    Anonymous said...  THIS is very common, our son was diagnosed (after much distress about it) w/ ADHD at 7...by the time he turned 10 we (his parents) had him evaluated for Apsergers...which we now realize he has. The problem w/ Aspergers is that teachers and most parents have no idea what it looks like. Only those trained or w/ experience notice the differences. The average age (which is way toooo late) a child is diagnosed w/ Aspergers is 10. Good luck on your journey!

(Especially in lower income areas) Doctors are more familiar with ADHD and tend to lean towards diagnosing that disorder in children who appear hyper (even if the behavior may be secondary to anxiety or another reason). Most doctors only see the child once or twice a year and most of the referrals to a psychologist is based on reports from schools and parents. Always do your own research and go into your doctors appointments prepared. Ask lots questions and always ask for alternatives. Healthcare in this country has a "turn and burn" mentality. They want to make an easy diagnosis with a simple plan of care. Make sure they aren't trying to fit your square child into a round hole.
•    Anonymous said... A lot of the behaviours overlap and sometimes it is hard to know which is which however my son has been diagnosed with both Aspergers and ADHD. Once he received medication for the ADHD it was easier to see the differences. Both are difficult in their own way.

After 3 yrs of following the school's push for meds for both, I went with my gut. He is meds free, and our family is so much happier. The meds made him very moody and irritable. Though he has a few "quirks", maturity and social classes have helped him to be more receptive of others, unless you are illogical.

Common for the two to sit together, there are alot of co-existing conditions, my eldest also has a dysgraphia diagnosis which is a form of dyslexia effecting writing. ADHD medication in a child that is not ADHD will make them climb the walls, in an ADHD child is has the opposite calming effect.

•    Anonymous said... Yes, our son also was diagnosed as ADHD and we struggled with that diagnosis for four years because he wasn't hyper. It was found later that his anxiety was actually the caliber of his diagnosis for mild autism (aka Aspergers). Four years we poured medication after medication into him trying to treat ADHD -- trying to find the right combination to no avail...no medications seem to work. It wasn't until we decided to intutionalized (the hardest decision we've ever made for him) so that doctors could review his medications and work with him. It was there, at this great facility, that we started to get better understanding about autism. Some children with autism...their anxiety is so high and debilitating that they can't function. That's what was going on with our son. When they started treating his anxiety, remarkable changes started to happen. All the big meltdowns, all the flight and flea activity, all the hitting and swearing started to diminish. I'm not saying that the anxiety medication has solved all of our problems, but it certainly has allowed our son to function and be able to control his actions. Aspergers can be extremely difficult to diagnose. Have you read "Aspergers for Dummies?" It's really great.

I was told for years my son had ADHD and totally by accident it was discovered he actually had aspergers with ADHD he was 9 when we found out

Hello, we had the same experience. Our boys got diagnosed 5 years ago with ADHD but even after that I kept thinking something still isn't right. We have gotten both diagnosed one with Asperger's the other with PDD-NOS. We are currently getting therapies for both and it is helping a lot. It doesn't hurt to keep investigating. Oh both the boys still take medicine for their ADHD and it still really helps. Good Luck

My son has Aspergers, diagnosed 2 years ago. He is now being assessed for ADHD. I've heard mixed opinions on this and will be interested to see the outcome of the assessment and answers to this question.

My son has a diagnosis of adhd and aspergers - they diagnosed adhd first but also suspected asd and so his doctor medicated him to supress the adhd symtoms to then be able to further assess him over a period of time via conors reports etc to assertain whether or not he had aspergers, which in his case he did. He is 9 now, he was diagnosed with adhd in March 11 then aspergers in Sept 11. :)

Apparently many children with Aspergers are incorrectly diagnosed with ADHD I always thought that was what my son had until I did a lot of reading....he definitely has Aspergers...was diagnosed officially in February.

Not uncommon at all, especially for boys! My son's first dx was ADHD & ODD. After 2 years of digging deeper, we finally got an Aspergers dx. His psychiayrist tells us thebehaviors that lead to the ADHD & ODD dx are all part of the Aspergers.

I would say absolutely...the first developmental ped. that my son saw said absolutely not on the spectrum, probably ADHD, but she didn't even diagnose that...called it developmental coordination disorder, sensory issues, along with assorted "quirks and variances"

As i read thru this sounds like the diagnosis my son has had adhd since he was 3 along with expressive/receptive delay, language disorder, attachment disorder, aggressive behaviors and just since May diagnosed with aspergers . The Dr wants to wait 3 years to actually diagnose with autism. It has been a whirlwind especially fighting with his dad about medication.

My girl was diagnosed with ADHD at age 7, then labeled with Asperger's at age 9. Now at 15 it has changed again to Sensory processing disorder and also Auditory processing disorder.

I have two sons diagnosed with adhd and then later...much later diagnosed asperger's ... one at 13 and one at 19. When they diagnose asperger's they usually drop off the ADHD diagnosis...I was told...and have experienced because an element of Asperger's has inattention and hyperactivity.

My son was diagnosed w/ Aspergers at 11 after going many, many years with diagnoses of ADHD, ODD, OCD, and others. Aspergers explains all of these and covers symptoms that these others do not. When we finally treated him as someone who as Aspergers, a lot of his behavioral and learning problems improved greatly.

Very common to have both disorders... Jacob was dx with ADHD at 5 but recently dx with aspergers too... I knew he was a bit different than the other kids I knew with adhd

My son was incorrectly diagnosed with ADHD, we battled against their decision because I knew it was wrong. I think they were confused by the hyperactive behaviour due to increased anxiety in stressful situations. We eventually saw another doctor and he agreed, good luck xxx

We were told our child was autistic at age 5. I didn't believe it because he was so talkative, but over the course of two years, the tics started and I started to notice him not playing with anyone. He was always argumentative to change, new situations, stressful situations, etc. He was evaluated by a private psychologist who said that he had asperger symptoms. We see the psychologist a couple of times a month now and he will start social skills training this year in third grade at school.

I just had my son diagnosed with Aspergers last yr . My son is 11yrs old now...he does have ADHD...last summer I did a med water on him and put him in a hospital to do it. That is when the Ped's doctor sent me to a Nero Dr and that is when we got the Aspergers diagnoses. without the medicines he is a very hyper boy and don't sleep....so we start Clonidine and ADHD medicine ...well we found out he can't ever take and stimualite medicine for ADHD....so he is on stratra and malitone for sleep helps alot .

We have had the same experience. My son was first diagnosed with ADHD and I thought that was really it for a little while but then it didn't explain other behaviors. Last year we got an Asperger's diagnosis. He also has severe anxiety and a bit of OCD! It gets overwhelming all the doctor appointments and trying to figure it all out. Meds help with some of the behaviors but they quit working sometimes it frustrating to and I know its harder on him.

Ding-ding-ding, we have a winner! Same with my son! ADHD at 6, Aspie at 11. Anxiety and depression as well as sleep problems thrown in there between those for good measure.
It is a constantly changing journey that will continue to change as he ages and hormones fluctuate. All we can do as parents is try to keep up and advocate the best we can...and LOVE them for the beautiful people they are!

definitely, same with my son and I. teachers and school nurses need to be educated in this because too many parents are being falsely accused of abuse in these circumstances, unfortunately even when proven innocent it sticks. We really need to campaign more for awareness as the lack of education has destroyed some families I know from authorities

My son was 3 when he was diagnosed with ADHD, at the age of 9 it was PDD NOS and at the age of 13 is now Aspergers.

Mine is OCD, ADHD and non-verbal learning disability. We took him to a wonderful neuro psychologist who was able to figure it out.

Initially thought my son was on the autistic spectrum with ADHD. Same experience as lady above - when get clear diagnosis of Aspergers they drop ADHD as symptoms come under aspergers anyway

Our son was diagnosed ADHD at 7, and finally confirmed as Aspergers at 10 even though we had known for a while. He is now diagnosed as both and we just give him meds to control the ADHD part of his brain that runs away with his thoughts. Luckily for us he is high enough functioning that he can pretty much be himself with the Aspergers as long as the ADHD is under control.

my son has both, he was first diagnosed adhd, But i KNEW there was more to it than that & the school, luckily, agreed with me hes now diagnosed adhd, & aspie! pleased to finally get diagnoses, sad that he has it. Love him to death x

It is very possible to have a duel diagnoses although there are traits in both that are the same..However they can be aspects of both that are the same. Not liking change, (this can manifest in my son when he's doing one thing and I ask him to do another). Not understanding friendships and the subtleties that are required to function well with 'friends'. I do not think my son has Aspergers however he definatly has traits. So I tend to parent him using both techniques that can be used with ADHD (which he does have) and Aspergers that he probably doesn't! I had to push to get my son ADHD diagnosed I knew that there was something not quite right. He never displayed the signs of being Hyper but has later developed these. Only later did I realise that some of his ways could also be classed as Aspergers. My Paeditrician said he is tricky to diagnosis as he doesn't show all the signs of ADHD although he has become more hyper as he's older (sigh!!!) he's 11 now.

My son was diagnosed as Asperger Syndrome 3 years age. He is 11 years old. Today psychologist talk about "hyperactive", but not ADHD (not yet). My son is "livid" all the time and I think this is the problem. This is problem with the emotion.

That is what my son was diagnosed with first. I knew it was more than that due to his anxiety and obsessive interests although 2 neuro psychs told me no. It took research and finding someone who was a specialist with Autism to get the correct diagnoses of AS. Go with your gut instinct. Good luck.

My daughter was Dx with both ADHD and Aspergers at the same time. When I first heard Doctors saying she was ADHD, I didn't believe it. I could see the aspie traits but not ADHD. Now that she is getting older, the ADHD traits are more pronounced. I didn't realize that would happen. Now I can't imagine what I would do without ADHD meds.

Yes, that's what happened in our case also. ADHD at 7 then ASD at 12. Definately has traits of both.

thats exactly what happend to us, we spent 6 years trying to find a DX, I KNEW he didnt have ADHD, it never felt right to me, we were blessed to get a refual to a specialist who dx'd him right away with aspergers, It of course wasnt news I wanted to hear, but it was a dx that felt more correct, and gave me something to go with and educate myself. Goodness the roller coster ride just won't stop, we've had the OCD, ADHD, ODD, depression, sleep issues, explosive disorder, and aspergers...o $ sensitivity issues... We've been on a few meds, focalin xr for awhile, now we are on concerta and a blood pressure med for the explosive, they help, but he still gets overwhelmed, depressed, and shuts down... We are in a much better place then we were two years ago... The journey continues...

This is us all day long, My son's 8th bday is tomorrow and I have been saying he has apserger's since he was 3 but the docs keep telling me no, but I am the only advocate for my son and I WILL NOT give up on him, we go the our local autistic diagnostic center on Aug 20th for testing. This site has truly changed our lives. To answer your question yes it is very possible we live it everyday! But as your child gets older you may see other changes as well.

This is their automatic diagnosis because it is easier to medicate the problems than figure them out.

My son has Aspergers, although the specialist said that it was a fine line between those two diagnoses.

My question is: if they're so similar, what difference does it make what the diagnosis is? Do the different categorizations afford access to different treatments/therapies/accommodations? I really want to understand this, as it's new for us.

As it is, I'm enrolling him in school this fall and letting them evaluate him for what they think he needs because I don't want him "labeled" from the get go. I figure I can use the specialists' reports when the school denies things like accommodations and adaptive technologies.

Apparently, DSMV doesn't allow High Funtioning Autism (Aspergers new name) and ADHD co morbidity, so my son's psychiatrist calls it Impulse Control Disorder, which is basically the same thing. In sessions, we refer to it as ADHD.

Getting the ADHD symptoms under control can help with the Aspergers, since being able to be patient and take the time to use tools instead of melting down when frustrated is crucial.

My son also has ODD and General Anxiety Disorder. Our psychiatrist is big on using diagnostic forms to determine as scientifically as possible how much the drugs are helping. We treat one symptom at a time, max out the benefit of the medication and then move on.

I'm just really grateful for good insurance.

I have a 16 year old son that was just diagnosed with High Functioning Autism a few months ago and all these years his pediatrician denied he had autism, always said he was ADHD. Finally, after emotional problems arose after his dad and I split and taking him to a psychiatrist, I was told he had HFA. His teachers have told me repeatedly over the years that it was not just ADHD but doctor still denied. Hopefully he can get the help he needs now better suited for him.

My daughter got diagnosed at 7 with Aspergers and two weeks later of ADHD when I asked what this ment had they wrongly diagnosed they said no miss brooks she as a duel condition this cleared things up as I already had a son with ADHD but she was so different I new it was something more only thought she get diagnosed with ADHD like her brother

My son was diagnosed at 5 with tourettes and ADHD. We knew he was having lots of anxiety and opposition and sensory so had him tested and he was diagnosed Aspergers at age 8. He did years of ot and pt and social skills and therapy. At he got older he has shown signs of depression and mood disorder but they haven't diagnosed bipolar. He keeps having these explosive meltdowns at 14 now. They are scary and he hurts people and destroys property. He has gotten worse. The newest counselor wanted information and his files about his diagnosis. I find that strange because why can't she just treat the issues we are having? Finding good therapy for these kids is next to impossible.

My child has been rejected by his peers, ridiculed and bullied !!!

How to Prevent Meltdowns in Children on the Spectrum
